THE FOLLOWING IS REQUIRED FOR PARTICIPATION IN BOYS LACROSSE IN THE SPRING AND ALSO FOR ANY OF OUR OUT OF SEASON ORGANIZED TEAM ACTIVITIES. DO NOT WAIT UNTIL THE LAST MINUTE. GET IT DONE EARLY-NOW!
 

1) 2026 Player Contact Information Form - Click HERE to complete the form - each spring season all players MUST Complete-Submit this form in addition to the completion of all below (note this form will be active-live closer to the start of the spring season). Use the new or incoming player contact form until the 2026 form is live, click HERE to be directed to it.

 

FCPS Mandated Documents for 2025-26 School Year:

 

All Student-Athletes Must Have a Physical Dated After May 1, 2024, Emergencey Care Cards and Health/Medical Insurance Coverage to Participate in Any WHS School Based Lacrosse Off-Season Organized Training Activities (OTA's) Beginning with the Summer Off-Season of Each School Year.

 

 

Westfield Team Based OTA's (Organized Team Activities): these are activities scheduled, coached and administered by the WHS lacrosse coaching staff on campus at Westfield

- Off-Season Sessions: our Summer-Fall-Winter activities
- Green Day Sessions (with full equipment): Lacrosse Based Skills-Drills Practice sessions

- Yellow Day Sessions (no equipment): Conditioning-SAQ (Speed-Agility-Quickness) and Strength Training sessions

 

- Although recommended, physicals are not needed to participate in any of the off-season camps (Commonwealth Camp) -tournaments - leagues (VEL) we participate in as a team. Basically anything off-campus out of season!

*All of the Following MUST BE completed-submitted no later than Friday, February 21, 2025

 - Submit completed paperwork to the Activities Office (place in envelope and address Boys Lacrosse) - slide under the door if it is closed-locked

 - All paperwork and concussion education requirements received after February 21, 2025 including on the Evening of Monday, February 24, 2025 prior to Practice-Tryout Session 1 will be considered late

2) 2025-26 Athletic Registration Form: This registration will allow you to complete the following for new and returning users: Emergency Care Card, Concussion Education, Weight Room Form, Drivers Form, Field Trip Form.....ALL of these are required of athletes.

3) PHYSICAL PAPERWORK: VERY IMPORTANT - PLEASE READ COMPLETELY
 
THERE IS A NEW FORM AND SUBMITTAL PROCESS FOR THE 2025-26 SCHOOL YEAR

2025-26 Physical:
 Must be Dated After May 1, 2025 - This is a new revised form and will be the only accepted for the school year. Please make sure it is filled out completely including all appropriate/required signatures (including doctors), insurance information and dated. If you've completed this for and played a fall/winter sport you DO NOT have to do this again, it should be on file with the school. If you have not played a fall or winter sport this school year, this MUST be completed and on file with the school prior to each student athlete participating in tryouts!

As our athletes get their physical completed they are to be submitted electronically directly to the athletic trainers. This allows you (the parents) to maintain a copy in your possession as well. If you do not have a scanner, the following is the iPhone scanning instructions that allow you to scan everything into one document.

 

Scanning to iPhone:

  1. Open notes app
  2. Click to add new note
  3. Click the paperclip
  4. Select Scan documents
  5. Scan all pages. Once you scan one page, click keep scan and continue until all pages are done.
  6. Save and attach to an email directed to slpartner@fcps.edu or rahartnett@fcps.edu. This puts the whole physical in one attachment!

Scanning to Android:

  1. Open notes app
  2. Create new note
  3. Click the plus sign in the top right
  4. Click scan
  5. Scan and click done
  6. Continue to scan until all pages scanned and hit save
  7. Click 3 dots in top right
  8. Click save as file (PDF file)
  9. Once saved, attach to an email and send directly to slpartner@fcps.edu or rahartnett@fcps.edu.
 
4) Emergency Care Card: please see #2 above, the athlete registration link to complete the ECC form.

5) Participation Policy Document:

There will no longer be a separate Participation Policy Document for the students to sign. The high school principals have worked to eliminate this document. Students are expected to adhere to all FCPS rules as stated in the Student Rights and Responsibilities Handbook. The consequences of any violation are also set forth in the SR & R. This includes missing activities for any violations.

 

 

6) Player-Parent Concussion Awareness Program: please see #2 above, athlete registration link to complete the Concussion Awareness Program as required by FCPS/VHSL.
 
Each participating student athlete and at least (1) parent-guardian must complete this course prior to the athlete's participation in an activity/sport. Highly recommended to complete this program prior to participation in the off-season activities (Green-Yellow Days). If you've completed this for and played a fall and/or winter sport this school year, you DO NOT need to complete this again. If you did not complete this for a fall and/or winter sport this school year, this MUST be completed and on file with the county prior to each student athlete participating in tryouts!
 
VIRGINIA CONCUSSION and BRAIN INJURY STATUTE Virginia Code 22.1-271.5 requires all student-athletes and a parent complete a concussion education program. Please click on the appropriate link below to begin the program. Upon successful completion of this program, the school will have your record updated. Students and parents must complete separate programs. Parents need only complete the program one time for all their children participating in FCPS athletics. THE FCPS CONCUSSION EDUCATION PROGAM is required for eligibility.

7) Health/Medical Insurance: All student athletes are required to have health/medical insurance to participate in athletics within Fairfax County Public Schools, including any and all out-of-season school and/or team based activities, including; weight training, conditioning and "green days".

 

Students and families without health/medical coverage can participate by purchasing School Accident Insurance online through K & K Insurance. This program may also serve as supplemental insurance for those who would like supplemental coverage to offset deductibles.

 

Please contact the WHS athletic training staff (Ms. Partner or Ms. Hartnett) should you have questions about the insurance requirement on 703-488-6413, otherwise, please use the following link as the School Accident Insurance through K&K Insurance must be purchased online. Upon purchase your receipt may serve as proof of insurance.

8) Weight Room Authorization: please see #2 above, athlete registration link to complete the Weight Room Authorization form.

 

All of our lacrosse players should have this form on file, it must be completed to participate in off-season workouts and in-season practice/tryout sessions.

 

*Strength training for the student athlete's is part of our YELLOW DAY Program (OTA's), ALL players please complete and submit for the off-season

 

**Even if a player is taking the Personal Fitness (strength training) Class, we require this form to be completed

 

 

Any-all password protected documents will be provided to the student athlete's accordingly.
